Development Services Decision Notice Request

This form can be submitted when you require a copy of a Development Services Decision Notice which is not available on PD Online.

Important Notes:

• In order to lodge the request, you must have the correct application reference number and property related to the Decision Notice you are requesting. Requests submitted without the correct Decision Notice and/or property cannot be processed.
• All requests for Decision Notices attract a processing fee, as outlined within Council’s fees and charges. Payment must be made, in full, prior to commencement of retrieving the Decision Notice. An officer from Council will contact you to advise of the amount payable.
• If you do not know the correct application reference number and property related to the Decision Notice, you will be required to obtain a Limited / Standard / Full Town Planning Certificate instead.
