1.All documents, reports and photographs relating to the burst water main in Macrossan Street, East Mackay.

2.All documents, reports and photographs relating to the failure of any main water line and water main servicing;

3.All assessor's reports and/or insurance reports relating to the above incident.

The following information is also requested:

(i)  All documents, complaint forms, report forms, incident reports, investigation reports, assessment reports, insurance reports and photographs relating to the failure of any water main lines in the surrounding streets for the 7 year period preceding the incident date;

(ii)  All maintenance schedules, repair schedules, records, work instructions and computer records reflecting all work undertaken by your company and/or your subcontractors to the water main lines in the surrounding streets for the 7 year period preceding the incident date;

(iii)  The precise date the water main lines located in the specified streets were last inspected (internally or externally) in the 7 year period preceding the incident date together with all documents, reports, internal memoranda or other documents relating to such works;

(iv)  The precise date the water main lines located in the specified streets were last replaced and/or upgraded in the 7 year period preceding the incident date together with all documents, reports, internal memoranda or other documents relating to such works;

(v)  All documents relating to any agreements Mackay Council has made with any party for the maintenance of the water mains.

(vi)  All documents relating to the relevant water industry standards in respect of the maintenance and/or replacement of water main lines.

(vii)  A map indicating the precise location of the prior incidents for the surrounding streets.

(viii)  Documents relating to the company policy, work instructions and/or formula Mackay council employs to determine when to replace and/or maintain a water main line.

(ix)  A large diagram and/or diagrams setting out the layout of all water main lines located within the surrounding streets, including information about the age, type and composition of the water main lines.

1. All email correspondence sent or received by Peter Cardiff in relation to the infrastructure contribution/charges and infrastructure agreements for The Waters Ooralea between 1 October 2010 to when Peter Cardiff left Council.

2. All email correspondence from the Mayor, CEO or Departmental heads in relation to:

a) Infrastructure charges; and
b) Infrastructure agreements

as referred to in paragraph 2.5 of your letter of the 19 December 2019, between the 1 March 2012 and 31 December 2012.

All documents confirming that the land included in the master plan development, known as “The Waters at Ooralea”, was accepted as being included in the urban footprint for the purposes of the Mackay Regional Planning Scheme, following a meeting held at the council on 26 May 2016. From, and including 26 May 2016, and including, 31 October 2016.

All property records, plans, strata, documentation, building reports, information relating to 21 Jackson Street, West Mackay QLD  4740 for the period of 1960 to 1965.

All details in relation to what Councillors were told in the month prior to 10 September 2019 and which Councillors approved of its removal from the beach with no community warning.

Copy of Engineers Report on or before 16 September 2019 of each pole of the Bucasia Swim Enclosure for time period July, August and September, copy of last maintenance work done on Bucasia Swim Enclosure for time period 2013 - 2019, copy of situation where poles removed from Bucasia Swim Enclosure are being held, storage documentation record for time period 16 August 2019 to present.

Copy of advice to the residents and public, and the time and ate of such publication of; advice of demolition of Bucasia Swim Enclosure and advice concerning road closures and changed driving conditions associated with the above action for the time period July, August, September 2019.
